Objective. The continuous delivery of oxygen is critical to sustain brain function, and therefore, measuring brain oxygen consumption can provide vital physiological insight. In this work, we examine the impact of calibration and cerebral blood flow (CBF) measurements on the computation of the relative changes in the cerebral metabolic rate of oxygen consumption (rCMRO2) from hemoglobin-sensitive intrinsic optical imaging data. Using these data, we calculate rCMRO2, and calibrate the model using an isometabolic stimulus.Approach. We used awake head-fixed rodents to obtain hemoglobin-sensitive optical imaging data to test different calibrated and uncalibrated rCMRO2models. Hypercapnia was used for calibration and whisker stimulation was used to test the impact of calibration.Main results. We found that typical uncalibrated models can provide reasonable estimates of rCMRO2with differences as small as 7%-9% compared to their calibrated models. However, calibrated models showed lower variability and less dependence on baseline hemoglobin concentrations. Lastly, we found that supplying the model with measurements of CBF significantly reduced error and variability in rCMRO2change calculations.Significance. The effect of calibration on rCMRO2calculations remains understudied, and we systematically evaluated different rCMRO2calculation scenarios that consider including different measurement combinations. This study provides a quantitative comparison of these scenarios to evaluate trade-offs that can be vital to the design of blood oxygenation sensitive imaging experiments for rCMRO2calculation.

Turfgrass managers rely on fungicides to suppress snow mold diseases in areas with persistent snow cover, which are commonly applied once or twice in late fall prior to snow cover. Fungicide applications targeting snow mold are expected to control snow mold diseases for the duration of winter; however, climate change is increasing the frequency of winter rainfall and snowmelts and may alter the duration of snow mold control. A 3-year study was conducted in Madison, WI to assess the impact of snow cover and antitranspirants on the persistence of the commonly used fungicides chlorothalonil and propiconazole. Snow cover, fungicide treatment, and the use of antitranspirants had minimal impacts on fungicide persistence and disease control. The most important factors influencing persistence of the fungicides were snowmelt and winter rainfall events, yet antitranspirants demonstrated modest evidence of extending chlorothalonil persistence in certain years. These results demonstrate that increasing snowmelt and winter rainfall events associated with climate change will increase dissipation of fungicides used for snow mold suppression which will likely impact disease control.

AIM: Determine the prevalence and define the profile of interlevel incidences (ININ) between primary care (PC) and hospital (HC). DESIGN: Multicenter cross-sectional descriptive study. SITE: Primary care. PARTICIPANTS: Professionals from a Health District and its reference hospitals. INTERVENTIONS: ININ are errors in communication between PC and HC professionals derived from administrative, pharmaceutical or clinical procedures not resolved during the formal interlevel communication processes, which requires a coordinated and validated response from the health care directions to not overload the family physician. MAIN MEASUREMENTS: ININ by category, hospital services and health centers, total and validated, relative to the total number of referrals, and the reason for the ININ. RESULTS: We detected 2011 ININs (3.36%) among the 59.859 referrals, although only 1684 were validated (83.7%). Most were administrative (59.5%), followed by pharmaceutical (24.2%), clinical (10.2%) and reverse (6.1%). 41.3% of the clinical ININs were grouped around 5 hospital specialties, and 45.9% in 5 health centers. The main reasons for clinical ININ were non-prescription of the recommended pharmacological treatment in outpatient clinics or on hospital discharge (27.3%), request for referral to another hospital specialist (27.9%), or request to referral in person to patients who had already been referred by teleconsultation (17.8%). CONCLUSIONS: 3.36% of interlevel referrals are accompanied by incidents and 83.7% are validated and processed. It is necessary to develop ININ management tools to guarantee safe healthcare and debureaucratize PC.

Antecedentes Diversos trabajos apoyan la hipótesis de que en España se está produciendo un aumento de incidencia de la escabiosis, y existen dudas sobre el posible desarrollo de resistencias y el incremento de formas clínicas atípicas. Los objetivos de este estudio fueron caracterizar el perfil demográfico y clínico de los pacientes de escabiosis atendidos por dermatólogos en España, identificar la posible aparición de escabiosis atípicas, así como describir la frecuencia y los posibles factores de riesgo de los fracasos terapéuticos previos. Métodos Realizamos un estudio observacional, transversal, multicéntrico con recogida de datos prospectiva, en abril y mayo de 2023 dentro de la plataforma CLINI-AEDVp de la Academia Española de Dermatología y Venereología. Resultados Se reclutaron 186 casos de escabiosis activa (51% mujeres) en 31 centros participantes de 15 comunidades autónomas. Se requirió un nivel A, B o C de los criterios de consenso de la International Alliance for the Control of Scabies (IACS) para el diagnóstico. El 92% de los pacientes presentaron formas clínicas típicas de escabiosis y un 66% había recibido tratamiento escabicida previo para el episodio en curso. De los pacientes previamente tratados, solo un 36% había recibido y cumplimentado una pauta terapéutica adecuada que incluyera el tratamiento simultáneo de convivientes, y un 50% careció de un documento escrito y claro con las recomendaciones. Conclusiones Una elevada proporción de los casos de escabiosis atendidos actualmente ha recibido tratamiento previo. En estos se observan defectos corregibles que pueden justificar parte de los fracasos terapéuticos. Trabajar en la mejora de las deficiencias encontradas ayudará a un mejor control de la enfermedad y a evaluar la efectividad actual de los escabicidas disponibles (AU)

Objetivo: Evaluar el comportamiento de los adenomas y las metástasis suprarrenales mediante TC con energía espectral, analizando el coeficiente de atenuación en imágenes monocromáticas a tres niveles energéticos diferentes (45, 70 y 140KeV), y la concentración tisular de grasa, agua y iodo obtenidos en los mapas de descomposición de materiales, con el fin de establecer puntos de corte óptimos que permitan diferenciarlos, y comparar nuestros resultados con la evidencia publicada. Materiales y métodos: Se diseñó un estudio retrospectivo de casos y controles que incluyó pacientes oncológicos con diagnóstico de metástasis suprarrenal en los 6-12 meses anteriores al estudio y con seguimiento en el Hospital entre enero y junio de 2020. Por cada caso (paciente con metástasis) incluido en el estudio se seleccionó un control (paciente con adenoma suprarrenal) con un nódulo de tamaño similar. Todos los pacientes fueron estudiados con un equipo de TC con intercambio rápido de Kilovoltaje, con protocolo de adquisición bifásico. Se analizó la concentración de iodo en el par iodo-agua; la de grasa en el par grasa-agua y la de agua en los pares agua-iodo y agua-grasa, tanto en fases arterial como portal. También se analizó el coeficiente de atenuación en imágenes monocromáticas (a 55, 70 y 140KeV) en fases arterial y portal. Resultados: En las imágenes monocromáticas, el coeficiente de atenuación fue significativamente mayor en el grupo de las metástasis que en el grupo de los adenomas en todos los niveles energéticos, tanto en fase arterial como en fase portal....(AU)

This enabled us to calculate the optimal cutoffs for classifying lesions as adenomas or metastases, except for the arterial phase at 55 KeV, where the area under the receiver operating characteristic curve (AUC) for the estimated threshold (0.68) was not considered accurate enough to classify the lesions. For the arterial phase at 70 keV, the AUC was 0.76 (95% CI: 0.663‒0.899); the optimal cutoff (42.4 HU) yielded 92% sensitivity and 60% specificity. For the arterial phase at 140 keV, the AUC was 0.94 (95% CI: 0.894‒0.999); the optimal cutoff (18.9 HU) yielded 88% sensitivity and 94% specificity). For the portal phase at 55 keV, the AUC was 0.76 (95% CI: 0.663‒0.899); the optimal cutoff (95.4 HU) yielded 68% sensitivity and 84% specificity. For the portal phase at 70 keV, the AUC was 0.82 (95% CI: 0.757‒0.955); the optimal cutoff (58.4 HU) yielded 80% sensitivity and 84% specificity. For the portal phase at 140 keV, the AUC was 0.9 (95% CI: 0.834‒0.987); the optimal cutoff (16.35 HU) yielded 96% sensitivity and 84% specificity. In the material density maps, in the arterial phase, significant differences were found only for the iodine-water pair, where the concentration of water was higher in the group with metastases (1018.8 ±â€¯7.6 mg/cm3 vs. 998.6 ±â€¯8.0 mg/cm3 for the group with adenomas, p < 0.001). The AUC was 0.97 (95% CI: 0.893‒0.999); the optimal cutoff (1012.5 mg/cm3) yielded 88% sensitivity and 96% specificity. The iodine-water pair was also significantly higher in metastases (1019.7 ±â€¯12.1 mg/cm3 vs. 998.5 ±â€¯9.1 mg/cm3 in adenomas, p < 0.001). The AUC was 0.926 (95% CI: 0.807‒0.977); the optimal cutoff (1009.5 mg/cm3) yielded 92% sensitivity and 92% specificity. Although significant results were also observed for the fat-water pair in the portal phase, the AUC was insufficient to enable a sufficiently accurate cutoff for classifying the lesions. No significant differences were found in the fat-water maps or iodine-water maps in the arterial or portal phase or in the water-fat map in the arterial phase. CONCLUSIONS: Monochromatic images show differences between the behavior of adrenal adenomas and metastases in oncologic patients studied with intravenous-contrast-enhanced CT, where the group of metastases had higher attenuation than the group of adenomas in both the arterial and portal phases; this pattern is in line with the evidence published for adenomas. Nevertheless, to our knowledge, no other publications report cutoffs for this kind of differentiation in contrast-enhanced monochromatic images obtained in rapid-kilovoltage-switching dual-energy CT scanners, and this is the first new contribution of our study. Regarding the material density maps, our results suggest that the water-iodine pair is a good tool for differentiating between adrenal adenomas and metastases, in both the arterial and portal phases. We propose cutoffs for differentiating these lesions, although to our knowledge no cutoffs have been proposed for portal-phase contrast-enhanced images obtained with rapid-kilovoltage-switching dual-energy CT scanners.

Generating pairs of Raman beams for gravimetry with fiber phase modulators is quite convenient but generates additional frequencies that must be filtered. The frequency filtering could be achieved by using a long (dispersive) birefringent calcite crystal followed by a polarizer that blocks the transmission of certain laser frequencies, as has been shown before. Here, we present a method to tune such a filter to the desired frequency position. The correction signal for the feedback is obtained by comparing (subtracting) the transmission through the filter when sending light that has been phase modulated or not, taking advantage of the fiber modulator that is already installed in the system. The method allows for continuously alternating between using the modulator for monitoring the filter position and other uses, an important characteristic for the operation of a complete gravimetric sequence.

El diagnóstico diferencial clínico entre los hemangiomas congénitos (HC) y los infantiles (HI) es complicado pero esencial para el tratamiento. El marcador inmunohistoquímico GLUT-1 ayuda a distinguirlos, sin embargo, la biopsia no es habitual. Se diseñó un estudio retrospectivo incluyendo los HI y a los HC diagnosticados en un hospital terciario en un periodo de 3 años, con el objetivo de describir y comparar los principales aspectos clínicos, epidemiológicos y terapéuticos. Se incluyeron un total de 107 hemangiomas, 34 HC (NICH/PICH/RICH), 70 HI y 3 pendientes de clasificar. El HI superficial de cabeza y cuello fue el tumor más frecuente. El tronco fue la localización más frecuente de los HC. Los factores de riesgo estudiados fueron más frecuentes en el grupo de los HI. Para los HI, el tipo de respuesta obtenida fue independiente de las variables (sexo, fecundación in vitro, profundidad, localización y tipo de tratamiento) (AU)


Distinguishing between congenital and infantile hemangiomas is challenging, but essential for appropriate treatment. The immunohistochemical marker glucose transporter type 1 is helpful, but biopsies are uncommon in this setting. Superficial infantile hemangiomas of the head and neck were the most prevalent tumors. Congenital hemangiomas were most often located on the trunk. Studied risk factors were more common in patients with infantile hemangiomas. Distinguishing between congenital and infantile hemangiomas is challenging, but essential for appropriate treatment. The immunohistochemical marker glucose transporter type 1 is helpful, but biopsies are uncommon in this setting. The aim of this retrospective study was to describe and compare epidemiological, clinical, and treatment characteristics of congenital and infantile hemangiomas diagnosed at a tertiary care hospital over 3 years. El diagnóstico diferencial clínico entre los hemangiomas congénitos (HC) y los infantiles (HI) es complicado pero esencial para el tratamiento. El marcador inmunohistoquímico GLUT-1 ayuda a distinguirlos, sin embargo, la biopsia no es habitual. Se diseñó un estudio retrospectivo incluyendo los HI y a los HC diagnosticados en un hospital terciario en un periodo de 3 años, con el objetivo de describir y comparar los principales aspectos clínicos, epidemiológicos y terapéuticos. Se incluyeron un total de 107 hemangiomas, 34 HC (NICH/PICH/RICH), 70 HI y 3 pendientes de clasificar. El HI superficial de cabeza y cuello fue el tumor más frecuente. El tronco fue la localización más frecuente de los HC. Los factores de riesgo estudiados fueron más frecuentes en el grupo de los HI.
